Passport Renewal and First-Time Passport Applicants

Please follow the instructions on this page and use form DS-11 if you answer YES to ANY of the below:

1.  You are applying for your first U.S. passport
2.  Your most recent U.S. passport was issued 15 years ago or more;
3.  You are replacing a lost or stolen passport
4.  Your most recent U.S. passport was issued when you were under 16
     years of age,

AND/OR:

a) your identifying information has changed (e.g. change of name); OR
b) your identifying information was printed with error; OR
c) your passport is valid for one year or less.

NOTE:

  • If you can submit your most recent passport, which was issued less than 15 years ago, and you were over 16 years old at the time of issuance, you are eligible to use form DS-82: Application for a U.S. Passport by Mail.
  • For children under age 16, please see Passports for Minors Under Age 16  for special instructions.

Please complete sections #1 to #22 on the application form. Incomplete forms will delay the issuance of your passport. DO NOT sign in section #23 as you must sign it in the presence of a consular officer at a U.S. Consular Service.

Submitting your application

You must submit your application in person at the Consular Office.

Processing time

It generally takes seven to ten days to process a passport application,
provided the application is in order on receipt at the Consular Section.

Passport Validity

Adults (age 16 or above): 10 years
Minors (age 15 or below): 5 years

DOCUMENTS TO BE SUBMITTED WITH YOUR APPLICATION

All documents submitted in support of your application must be either originals or copies certified by the state or federal authorities, bearing the official's seal and signature, and must not be laminated. Hospital records/certificates will not be accepted. Photocopies of documents notarized by a Clerk of Court, Justice of the Peace, solicitor or Notary Public are not acceptable. An official written translation is required for any documents issued in languages other than English. The submitted documents will be returned to you with your passports.

1. Evidence of U.S. citizenship:

a) Your most recent U.S. passport; OR
b) If you are applying for your first passport, submit one of the
     following:  

  • Birth certificate if you were born in the U.S.; (hospital records are not acceptable)
  • A Consular Report of Birth Abroad (form FS-240) or Certification of Birth (form DS-1350 or form FS-545) if you were born abroad to U.S. citizen parent(s);
  • Certificate of Naturalization, or Certificate of Citizenship.

2. Evidence of identity

Valid identifying documents containing your current name and  
signature along with a physical description or photograph. Examples: 
foreign passport, Certificate of Naturalization, driver's license, etc. If 
you are applying for your first U.S. passport or if your facial image
and/or signature have changed since your last passport was issued,
please submit progressive identifying documents from your childhood
to the present (e.g. school yearbooks, student ID card, old passports,
old driver's license, etc.).

3. (If applicable) Evidence to support change of the descriptive data
      on your passport

a) If your name has changed since your most recent passport was
issued, you must submit a marriage certificate; a court order;
divorce decree; or a certificate of change of name, to support the 
change of name. These documents must show the name on your 
current passport AND the name you are requesting to change to. 
A valid identifying document (e.g. driver's license, foreign passport)
bearing your new name and signature should also be presented if 
available.

b) If there is a change or an error in the descriptive data in your
recently issued passport, you must submit the appropriate evidence
showing the correct information (e.g. birth certificate, marriage
certificate).

a) If your most recent passport was issued with limited validity due
to lack of evidence of citizenship and/or identity at the time of its
issuance, you must submit the necessary documents as advised by the
issuance authority (i.e. U.S. Embassy, Consulate General, the Passport
Services in the U.S.).

4. One recent color photograph taken full face, with a white background, measuring 5 cm by 5 cm (2 in x 2 in). See our photo requirements

Return or collection of your passport

The passport submitted with your application will be cancelled and returned to you with the new passport. If you do not wish to collect them in person at the U.S. Consular Office, passback can be arranged with the private courier company UPS. Otherwise, you must collect your passport in person at the U.S. Consular Office where you submitted your application.
